Joint/Dual life claim forms

This form is to be completed when there are two people covered under the plan. This claim form is to be completed by the second person covered under the plan or by the trustee or assignee.

Single Life claim forms (life cover under €60,000)

This form is to be completed where there is one person covered under the plan and the life cover amount is less than €60,000. The claim form is to be completed by the next of kin or executor.

Single Life claim forms (life cover greater than €60,000)

This form is to be completed where there is one person covered under the plan and the life cover amount is greater than €60,000. The claim form is to be completed by the executor or administrator.

This form is to be completed by the plan owner.
