Reinstating a lapsed plan
To reinstate a lapsed plan, you must complete the evidence of health form below and return to us. Final decision on whether we are in a position to reinstate your plan will rest with our Underwriters.
Changing direct debit details
If you would like to change the bank account details or start making payments by direct debit, please fill in the direct debit mandate below and return to us.
For your convenience Irish Life will process any changes to your direct debit when using either the Direct Debit Mandate or the Withdrawal Forms. There is no need for you to contact your bank
